Application form “ΑΙΤΗΣΙΣ” cancelled by pen “περί τροποποιήσεως της επιγραφής, ή αλλαγής του Γραφείου προορισμού, ή αναστολή πληρωμής επιταγής” (Application to amend the inscription, or change the Post Office of destination, or suspend payment of a money order), franked with 5 δρχ (Hercules Labors issue) stamp, tied with “ΝΙΚΑΙΑ - ΕΠΙΤΑΓΑΙ* 17 IV 72” postmark, plus “ΑΘΗΝΑΙ - ΕΠΙΤΑΓΑΙ* 14 IV 72 postmark, caption “ΥΠΗΡΕΣΙΑ ΕΠΙΤΑΓΩΝ” and printed form code “Α.Μ. 1651”. Because the beneficiary was illiterate, 2 witnesses signed for him. VF and Scarce.
NOTE. Two types are known of above application form:
1. One with caption “ΥΠΗΡΕΣΙΑ ΕΠΙΤΑΓΩΝ” (Service of Checks) and printed form code “Α.Μ. 1651”.
2. One with caption “ΕΛΛΗΝΙΚΑ ΤΑΧΥΔΡΟΜΕΙΑ” (Hellenic Posts) and without printed form code.
